Truck Financing Application

We will use this form to submit your information to one or more independent financing companies. These financing companies may require additional forms or information.

Please complete all sections, sign and date the form, and forward this application to our office. Your application can usually be processed within 24 hours.
NJ ST-4 Sales Tax Exemption Certificate
Replacement parts purchases for commercial motor vehicles registerd in New Jersey for more than 26,000 pounds are exempt from New Jersey sales tax, if an appropriate NJ ST-4 Form is on file with the seller. Please complete all sections, sign and date the form, and forward the certificate to our office. Please be sure to complete "purchaser's NJ certificate of authority number" which is your federal tax identification number.
NJ ST-3 Resale Certificate
If your company resells merchandise purchased from H & H Mack Sales and collects sales tax on those sales, you may qualify for exemption from NJ sales tax for your purchases if a valid NJ ST-3 Certificate is on file with the seller. Please complete all sections, sign and date the form, and forward the certificate to our office. Be sure to complete "purchaser's NJ certificate of authority number" which is your federal tax identification number.
